MBLEx Exam Requirements: Who Can Take the MBLEx?
To take the MBLEx, you need three things: a complete application with the $265 fee, your massage education verified directly with the Federation of State Massage Therapy Boards (FSMTB), and your agreement to follow FSMTB's exam policies (2026 Candidate Handbook, pp. 5–6 (PDF)).
You don't always have to graduate first. FSMTB also accepts students who are still enrolled in an approved program, as long as they've already received education and training in every subject area of the MBLEx Content Outline. Your state may separately require graduation, a specific number of education hours, or other conditions for a license.
Which path are you on?
Find your situation, then do what's in the last column. Every row also needs the application, the $265 fee, and agreement to FSMTB policies.
| Your situation | What FSMTB needs before it approves you to test | Your next move |
|---|---|---|
| Graduated from an approved massage program | Your program verifies your education directly with FSMTB through the Education Verification Center (EVC) | Apply, then tell your school you applied so it completes the verification |
| Still enrolled in an approved program | You've already received education and training in all MBLEx content areas, and your program verifies that through the EVC | Ask your school whether it will verify you now, then apply |
| Trained through an apprenticeship | Your state board confirms it accepts your apprenticeship for licensure. Your mentor or sponsor sends your records directly to FSMTB. If you've completed the apprenticeship, you must apply for state licensure before FSMTB approves you | Contact your state board first |
| Trained outside the U.S. | An independent equivalency evaluation of your training, with evaluation documents in English. You must apply for state licensure before FSMTB approves you | Check whether your state requires a specific evaluation agency, then start your state application |
| Your school has closed | Your records come to FSMTB from the closed school's records custodian or repository, not just from you | Find the records custodian and ask for records to be sent directly to schoolsupport@fsmtb.org |
| You passed before, and a state requires you to retest | Written approval from that state regulatory board or agency | Get the state's written approval before you reapply |

If you're on the ordinary school path, the step that most often holds things up is the school's verification. FSMTB says making sure it happens is your responsibility. Documents submitted only by you don't complete the education-verification requirement unless FSMTB specifically instructs otherwise.
What counts as an "approved" program?
A program is approved if it is approved or recognized by the state board or agency authorized to regulate massage therapy where the school is located. If that massage board or agency does not approve or recognize education programs, FSMTB determines whether the program meets MBLEx eligibility requirements (2026 Candidate Handbook, p. 6 (PDF)).
Is there a minimum number of school hours to take the MBLEx?
FSMTB's current eligibility policy does not state a universal minimum-hour threshold for sitting for the MBLEx. Instead, it requires either graduation from an approved massage therapy education program or enrollment in an approved program plus education and training in every MBLEx Content Outline subject area.
Your state licensing board may separately require a specific number of education hours or completed graduation before it will issue a massage license. Check the board in the state where you plan to practice.
FSMTB also says it does not restrict how much time may pass between graduation from an approved program and applying for the MBLEx (FSMTB MBLEx FAQs).
How to apply for the MBLEx
- Read the Candidate Handbook. FSMTB requires applicants to review the current handbook before applying (2026 Candidate Handbook (PDF), effective September 17, 2026).
- Create your FSMTB Examination Account and complete the application. Enter your name so it matches the identification you'll present on test day.
- Pay the $265 fee. Online payments use Visa or Mastercard. FSMTB also accepts certified checks and money orders in U.S. dollars and does not accept personal checks. The application fee is nonrefundable and nontransferable (2026 Candidate Handbook, pp. 7–9 (PDF)).
- Request testing accommodations during the application process, if you need them. Test-center staff can't approve accommodations on exam day. FSMTB does not charge an extra accommodations fee and may request additional documentation when needed (2026 Candidate Handbook, p. 8 (PDF)).
- Get your education verification in. Tell your school you've applied so it can verify your education through the EVC. Your submitted application is valid for six months; if it is still incomplete when that period ends, it expires and you must submit a new application and fee (2026 Candidate Handbook, p. 11 (PDF)).
- Wait for your Authorization to Test (ATT). FSMTB says it takes five business days to process a complete application. A complete application consists of the application, fee, and education verification (2026 Candidate Handbook, pp. 6 and 11 (PDF)).
- Schedule with Pearson VUE using the instructions on your ATT. Pearson uses its own scheduling system, separate from your FSMTB Examination Account. The MBLEx is taken at a Pearson VUE professional testing center, not online at home (FSMTB MBLEx FAQs).
Veterans may be able to seek reimbursement of an approved licensing or certification test fee through applicable VA education benefits. Check the VA's licensing and certification test-fee information for current eligibility.
Your Authorization to Test (ATT)
The ATT is FSMTB's approval that lets you schedule the MBLEx. FSMTB emails it after your application is approved, and it also appears in your FSMTB Examination Account.
Your ATT shows the dates you're allowed to test. FSMTB's current FAQ describes the window as 120 days, but the dates printed on your ATT control your individual authorization (FSMTB MBLEx FAQs; 2026 Candidate Handbook, pp. 9 and 16 (PDF)).
If you don't test before the expiration date on the ATT, it expires and you must reapply as a new applicant. If you arrive late and miss your appointment, FSMTB treats you as a no-show, your ATT expires, and reapplication is subject to the current application and fee requirements.
ID requirements on test day
Bring two IDs: one primary and one secondary. The format and spelling of your name must match your MBLEx application, your ATT, and the identification you present. Your middle name or initial is not required, but your first and last names must match exactly. If they don't, contact FSMTB before exam day (2026 Candidate Handbook, p. 15 (PDF)).
| ID type | Requirements | Examples from the 2026 handbook |
|---|---|---|
| Primary ID | Original, physical, valid, unexpired, government-issued photo ID. If it has a signature line or field, it must be signed. A qualifying government-issued photo ID without a signature field can still be accepted. | Driver's license; state/territory ID; learner's permit or instruction permit if issued as a qualifying physical photo card; passport or passport card; U.S. Certificate of Naturalization; military ID; Permanent Resident Card/Green Card; Employment Authorization Document |
| Secondary ID | Original, valid, unexpired ID showing your name and signature | Signed Social Security card; signed employee/work/school ID; signed bank-issued credit, debit, or ATM card; or another qualifying primary ID if it contains a signature |
| Not accepted | Documents or images that cannot reliably verify identity | Photocopies; screenshots; photos of ID; printed DMV receipts; renewal paperwork; paper temporary licenses or IDs; expired or invalidated IDs |
Digital IDs are not accepted as standard MBLEx practice. Pearson may accept a qualifying government-issued digital ID only where applicable law requires acceptance and Pearson has completed its required validation process.
Plan to arrive at least 30 minutes before your scheduled appointment. Cell-phone access is prohibited while you're at the test center, including during breaks; store prohibited personal items as directed by the center (2026 Candidate Handbook, pp. 18–19 (PDF)).
Exam requirements vs. license requirements
Passing the MBLEx doesn't license you to practice. FSMTB determines exam eligibility and administers the MBLEx; the state regulatory board or agency determines whether you meet its licensing requirements and issues the license.
For many candidates, the state-license application happens after the MBLEx. But that timing is not universal: applicants with completed apprenticeships and applicants educated outside the United States must apply for state licensure before FSMTB will approve them to take the MBLEx.
| Requirement | To take the MBLEx | To get a massage license |
|---|---|---|
| Education | Graduate from an approved program, or be enrolled and already trained in every MBLEx content area; special apprenticeship and international pathways also apply | State-specific education, completion, hour, and documentation rules |
| Proof | Education verification goes directly to FSMTB through the applicable pathway | Whatever the applicable state board requires |
| Fee | $265 MBLEx application fee | Separate state licensing fees may apply |
| Who decides | FSMTB | State regulatory board or agency |
For current licensing requirements, use the FSMTB Regulated States directory to find the applicable board, then confirm the rule on that board's own site. FSMTB warns that some states are changing education-hour requirements, so the controlling state source should be checked for an actual license application.

Common questions
Is there a 500-hour rule to take the MBLEx?
Not from FSMTB as a universal exam-eligibility rule. FSMTB's current policy looks at whether you graduated from an approved program or are enrolled and have received education and training in all MBLEx Content Outline subject areas.
A state may separately require 500 hours, more than 500 hours, or another education standard for licensure.
Do I apply to my state board before the MBLEx?
On the ordinary approved-school path, FSMTB does not make a prior state-license application part of the standard MBLEx eligibility rule. Applicants with completed apprenticeships and applicants educated outside the United States do have to apply for state licensure before FSMTB approves them to test.
Either way, check your state's licensing requirements early so you know whether your education meets the requirements for the license you ultimately want.
Can I retake the MBLEx if I fail?
Yes. You must submit a new application and pay the fee in effect at the time. FSMTB does not require a waiting period before you reapply and does not limit the number of MBLEx attempts, but your state regulatory board or agency may impose its own attempt limit (2026 Candidate Handbook, p. 23 (PDF); FSMTB MBLEx FAQs).
If you've already passed the MBLEx and a state later requires you to retest, you need that state regulatory board or agency's written approval before submitting a new MBLEx application.
Can I take the MBLEx online at home?
No. FSMTB says the MBLEx is not available as an at-home online exam. You take it at a Pearson VUE professional testing center (FSMTB MBLEx FAQs).
